背景
急性早期 HIV 感染患者传播 HIV 的风险高于慢性感染患者，但是有关早期感染对于 HIV 传播的作用还存在争论。 我们在利隆圭和马拉维调查了早期感染在 HIV 发病中的作用，并假定只对早期感染、只对慢性感染以及对两者同时进行预防干预，以预测其远期效应。
方法
我们利用在利隆圭收集到的详细行为以及病毒载量的数据，开发了一种确定性数学模型以描述异性恋中的 HIV 传播。 我们纳入了稳定的伴侣之间或之外的性接触，并将传染期分成了若干阶段，以便发现各感染阶段中传播性的变化。 我们使用贝叶斯融合法将模型对 1987 年至 2005 年间在利隆圭产前诊所收集的艾滋病毒感染率数据进行拟合。 我们评估了能够使人群中每次性接触的传播概率降至 0.00003 的干预措施，并且改变每个阶段接受干预措施的人数比例。
结果
我们预计在利隆圭有 38.4% （95% 置信区间为 18.6-52.3）的 HIV 传播是与早期感染者发生性接触所导致的。 仅针对早期感染的干预措施能大幅降低 HIV 的发病率，但即使对 100% 人群进行此项干预也不能根除发病。仅针对慢性感染的干预措施也能够降低 HIV 的发病率，但是需要有 95%-99% 覆盖率才能根除 HIV 发病。 当仅针对慢性感染的干预措施覆盖率低于 95% 时，对 25-75% 的早期感染者进一步实施干预能大幅降低 HIV 的发病率。
解释
我们的结果提示在撒哈拉以南的非洲地区，早期感染对 HIV 传播具有重要作用。如不能达到 100% 覆盖，针对慢性感染的干预措施可能达不到预期效果，但可通过针对 HIV 早期感染者的干预措施来弥补。
