GRE Admission Ticket

• You must bring this admission ticket, along with acceptable and valid photo identification with signature, to be admitted to the test center.
• To correct your personal data, make address changes, or to add, delete, or change score recipients, please see the correction stub below. The completed stub must reach ETS by the Friday before the test date.

General Information

• Take three or four No. 2 or HB (soft lead) pencils and an eraser.
• Mechanical pencils and pens of any kind are not permitted in the testing center.
• No one will be admitted after testing has begun.
• Rescheduling Your Test ($50): Contact the GRE Program no later than 3 full days before your scheduled test date (not including the day of your test or the day of your request) or your test fee will be forfeited. Include your name, date of birth, address, registration number, test date, preferred test center, and new test date. You cannot reschedule to the next testing year.
• Canceling Your Test: If you contact the GRE Program no later than 3 full days before your test date with your name, date of birth, address, registration number, and test date, you will receive a refund equivalent to half of the original test fee. Allow 6-8 weeks for receipt after the canceled test date.

GRE Correction Stub Instructions

• If there are any errors on your admission ticket or the correction stub, print this page and write the correct information in the space directly below the incorrectly printed items. Name changes are not allowed. Address corrections should be made in the space provided.
• Also shown are the undergraduate institution code and graduate institution codes you indicated.
  • To delete a score recipient, print the word "DELETE" in the space under the code.
  • To add a score recipient, enter the institution’s code number (from the code list at www.ets.org/gre/stupubs) in any unused set of boxes.
  • To change a score recipient, cross out the code number and enter the new score recipient’s code number under the crossed-out code.
  • A plus (+) means a number is invalid or inactive. Check the code list (at www.ets.org/gre/stupubs). If the code number is correct, the indicator means that the institution does not receive GRE scores.
  • If an institution you wish to designate is not listed, write in the full name and address of the institution. If the institution is authorized to receive GRE scores, your request will be honored.
• If adding or changing a score recipient, enter the code for the institution and/or department and indicate the scores you want reported.
• Score Report Codes: G = General Test scores, S = Subject Test scores, N = No scores (undergraduate institution only). If you do not indicate a score report, ALL SCORES WILL BE REPORTED.
• If you have made changes, detach the stub at the dotted line and mail to ETS-GRE, PO Box 6000, Princeton, NJ 08541-6000; or fax to 1-610-290-8975, GRE–Mail Stop 02-Q. You may also e-mail your request to gre-info@ets.org. Include all identifying information.
• IMPORTANT: Your corrected stub must be received at ETS in Princeton, NJ, no later than the Friday before the test date printed on it. Do not send a letter with this form.
